On this page, we have made available all the forms required for the various activities at Nazarene Christian School. These do not guarantee admittance to any of the NCS Programs- they are merely here for you to print out and complete before the interview/registration process.

You may print these at your convenience and bring them in with you or you may complete one at the school.
